Join us for the annual meeting of the local and Tribal Washington assessment community.
During this annual meeting, you will participate in interactive training and workshops to build skills in areas including CHAs and CHIPs, Tribal community engagement, communicating with data visualization, and other topics relevant to assessment.
The meeting kicks off at 11:00 am Pacific time on Monday, May 6, and wraps up at noon on Wednesday, May 8. Registration is no longer limited and we welcome additional registrations from all jurisdictions until we reach capacity. Lodging
Foundational Public Health Services (FPHS) will reserve lodging directly and pay for room rate + taxes/fees at one of three hotels. Reservations for participants registered by April 4th will be at a reimbursable rate at one of the following hotels on a space-available basis. If participant(s) will share rooms, each participant needs to indicate name(s) of roommate(s) when registering.
If you will travel on and need lodging for May 5th, we have a limited number of rooms at the block book rate of $ 107/night + taxes and fees. You must reserve your own lodging for this night and do so directly with the hotel and with a personal credit card. Our organization will pay hotels directly for May 6th and 7th; May 5th costs are your personal responsibility and will not be reimbursed. To reserve and be billed for May 5th (Only), contact the following person and mention Statewide Assessment Meeting and all nights you plan to stay. Travel Expenses and Reimbursement
FPHS funding will reimburse personal vehicle mileage for two-way travel (doorstep to location), meals while in state travel status and not provided at the event nor included in hotel rate, and hotel parking. For those with special mobility needs, reimbursement for ground transportation to and from hotel and/or event parking may be possible. Direct questions to nwcphp@uw.edu.
To request travel expense reimbursement, complete and send by June 7, 2024 the Non-employee travel form CURRENT.pdf (uw.edu) and required receipts (.pdf preferred) to nwcphp@uw.edu.
Photo Release
The Northwest Center for Public Health Practice (NWCPHP) may take photos and videos during this
training for use in its educational and promotional materials. By virtue of registering, you grant NWCPHP
full rights without compensation to use any photos, videos, or recordings containing your likeness taken
during the routine course of the event by NWCPHP or its official representatives to be used in future
educational or promotional endeavors of NWCPHP, the School of Public Health, and the University of
Washington, without any further notice. This permission may be revoked in writing to NWCPHP.
Survey Data
By registering, you agree that NWCPHP may use your de-identified, aggregate survey responses to inform
improvements to this training and the Center’s future training plans, as well as share this information with
funders.
